We thought that it would appropriate to wind down our series on Medicaid by providing our readership with a comprehensive listing of where you can go to find a Medicaid application. Medicaid is a program that is jointly funded by the federal and state governments, but administered through state agencies - so that means were offering you a complete list of all 50 health services agencies here in the United States so you can find state specific applications.

Applying for Medicaid

As a quick reminder, Medicaid is different than Medicare - which provides qualifying individuals with health and hospital insurance, more commonly referred to as Medicare Part A and Medicare Part B as well as Medicare Part D, which is for prescriptions. Medicaid, on the other hand, is a program that helps five broadly defined groups including children, pregnant women, adults with dependent children, individuals with disabilities, and those age 65 and older.

Medicaid eligibility is determined by each state, using guidance provided by the federal government. Generally the program is reserved for America's poorest individuals. Eligibility is usually determined by each state using a combination of income and assets.
